Companion / Personal Support Position Overview: APD CERTIFIED A MUST OR must be an EXCEPTIONAL APPLICANT
Direct support staff play a critical role is supporting adults with diverse abilities by assisting with daily activities, life skills development, and community integration. Key Responsibilities:

Provide direct care and support to individuals in daily activities, including community inclusion personal care, meal assistance, and hygiene support.
Incorporate client's goals as stated in the Support Plan.
Encourage engagement, inclusion, and social interaction in the community.
Monitor and document participant progress, behavior, and health status in a clear and timely manner daily into the I connect system. Failure to enter notes into I connect will result in termination or withhold of pay until notes are entered.
Communicate effectively with team members, participant, and families to ensure high quality care.
