Community Health Care Coach

Salary: $39,000

Job Overview: Elder Options is seeking a Community Health Care Coach, to provide community health education, follow up support and coaching to patients.

Key Responsibilities: Services include Care Transition services supporting hospital to home transitions and/or other community-based health and wellness services. The interventions are geared toward patient self-management of chronic diseases via teaching/modeling, self-advocacy, and education with the purpose of reducing re-hospitalizations, providing individualized assessments, care planning, referrals and supports to adults with health and wellbeing goals to support their independence in the community. Home visits are required and remote work is limited.

Minimum Qualifications: Bachelor’s degree (B.A./B.S.) from four-year college or university or equivalent experience in providing human services.

Required Skills and Abilities: Successful completion in the Take Charge Care Transition Model is required for maintaining position.

While performing the duties of this job, the employee is regularly required to talk and hear. The employee is occasionally required to stand, walk, climb, or balance; and stoop, kneel or crouch. The employee must occasionally lift and/or move up to 20 pounds.

Preferred Qualifications: Experience working with older and/or disabled adults and Spanish-speaking preferred.
