SUMMARY: The Case Manager provides assessment, monitoring, planning, linkage and advocacy for the most appropriate services for individuals and families enrolled in rapid rehousing services so they may access permanent housing and achieve housing stability. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for individuals and families experiencing homelessness.
ESSENTIAL DUTIES AND RESPONSIBILITIES: (These essential job functions are not to be construed as a complete statement of all duties performed. <li>Screening households to determine and document eligibility.</li>
<li>Assessment of the individual and other family members of their strengths, needs, abilities and preferences to assist in the development of housing goals.</li>
<li>Ability to focus on housing and to use strengths- based practices focus on participant engagement and meeting the unique needs.</li>
<li>Developing a Housing Stability Plan, addressing crisis intervention, potential barriers, housing identification, move-in, self-sufficiency and budget for each household within one week of entry into program.</li>
<li>Monitor progress daily and maintain accurate documentation of progress towards goals and services provided.</li>
<li>Monitor participants housing stability and be available to resolve crisis, at a minimum during the time rapid-re-housing assistance is provided.</li>
<li>Work directly with Coordinated Entry System.</li>
<li>Providing referrals and facilitating access to services and community resources as needed.</li>
<li>Providing on-going risk assessment and safety planning.</li>
<li>Conduct home visits.</li>
<li>Responsible for collecting all necessary documentation for client files according to established guidelines.</li>
<li>Process requests for rental assistance, application fees and other forms of assistance.</li>
<li>Utilize HMIS and coordinate with the Coordinated Entry System.</li>
<li>Attends regular meetings of RRH providers.</li>
<li>Adheres to countywide RRH policies and procedures.</li>
<li>All other duties as assigned.</li>
<li>Participates in Agency Performance Quality Improvement (PQI) program and Accreditation/Reaccreditation process.</li>
<li>Mission-driven attitude supplemented with integrity and passion.</li>

<li>This position requires a minimum of a bachelor’s degree in social work or related field.</li>
<li>Minimum 2-years’ experience serving homeless or at-risk families and/or individuals in crisis.</li>
<li>Must be familiar with a multitude of wrap around services and providers within the CoC.</li>
<li>Must be able to comply with complex governmental regulations, policies and procedures and demonstrate thorough document compliance efforts and activities.</li>
<li>Must be proficient with data management and information systems and have basic knowledge of Excel, PowerPoint, and Outlook.</li>
