Key Responsibilities

<li>Evaluate patients’ speech, language, and hearing abilities, recommending strategies to enhance communication and hearing.</li>
<li>Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.</li>
<li>Collaborate with interdisciplinary team members on care plans and provide speech pathology training to team members as needed.</li>
<li>Communicate regularly with physicians, addressing any changes in patient condition.</li>
<li>Participate in the periodic revision of care plans, updating the physician and care team on patient status and obtaining additional orders as required.</li>
<li>Educate patients and families/caregivers on speech therapy techniques and related challenges.</li>
<li>Confirm daily visit schedules to ensure seamless coordination with other staff.</li>
<li>Document all evaluation data, treatments, and patient progress promptly.</li>
<li>Supervise home health aides every 14 days when speech therapy is the sole discipline involved.</li>

Qualifications 
<ul>
<li>Must have a current SLP license (state-specific).</li>
<li>Graduate of an accredited speech language pathology program.</li>
<li>Valid BLS certification.</li>
<li>Minimum two years of speech therapy experience (home health experience preferred).</li>
<li>Excellent communication, organizational, and time management skills.</li>
<li>Must have a valid driver’s license and access to a reliable and insured vehicle.</li>
<li>Ability to visit homes that may not be wheelchair accessible.</li>
<li>Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.</li>
<li>Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.</li>

Work Environment

Employees will have a 30-minute lunch and two 15-minute breaks, which are scheduled and not flexible.

GENERAL SUMMARY:

<li>Verifies patient information by interviewing patient, recording medical history, and confirming purpose of visit. Accurately records all information in EMR system according to established standards and physician requirements.</li>
<li>Checks condition of patients' eyes by observing pupils, muscle, visual acuity, extraocular movements, and blood pressure (if requested by Physician).</li>
<li>Prepares patients for ophthalmology examination by dilating pupils, changes in visual acuity, elevated extraocular pressure, or blood pressure and communicating results to Attending Physician.</li>
<li>Secures patient information and maintains patient confidence by completing and safeguarding medical records, completing diagnostic and procedure coding, and keeping patient information confidential.</li>
<li>Counsels patients by transmitting physician's orders, use of drops, contact lenses, drugs, and answer questions about surgery and specialty imagining required/ordered.</li>
<li>Maintains safe, secure, and healthy work environment by establishing and following standards and procedures along with complying with legal regulations.</li>
