Job Summary
We are seeking an experienced Travel Social Worker to support patient care coordination, discharge planning, and psychosocial assessments in a healthcare setting. The ideal candidate will have strong clinical social work experience and be comfortable working in a fast-paced hospital environment.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Coordinate discharge planning and post-acute care services
- Complete PASRR evaluations and ensure compliance with regulatory requirements
- Collaborate with physicians, nurses, case managers, and interdisciplinary care teams
- Document patient information accurately using hospital systems
- Communicate effectively with insurance providers and post-acute facilities
- Advocate for patients’ social, emotional, and care transition needs
Required Qualifications
- Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW)
- Experience with MIDAS / Meditech systems
- PASRR experience required
- NaviHealth experience required
- Previous hospital or acute care social work experience preferred
- Strong communication, documentation, and care coordination skills

- Travel position – flexibility required
