Bilingual Social Worker Hours
- M-F 8am-5pm
- Must reside in Hillsborough County, FL, and within 2-3 miles of the 33624/33625 zip code.
- Candidates will travel approximately 75% of the time within the region to see members.
- Training will be conducted remotely via Microsoft Teams.
Bilingual Social Worker Compensation
- The pay for this position is $34.13 an hour
- Benefits are available to full-time employees
- A 401(k) with a company match is available for full-time employees.
Bilingual Social Worker Responsibilities
- Coordinating case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
- Assessing, planning, implementing, and coordinating all case management activities to evaluate the medical needs of members.
- Managing a caseload that includes supportive and medically complex members.
- Determining appropriate services and supports based on members' health needs.
- Utilizing care management tools to enhance member wellness and outcomes.
- Conducting multidisciplinary reviews to achieve optimal healthcare outcomes.
- Engaging and empowering members to participate in their healthcare decision-making.
Bilingual Social Worker Requirements
- Bachelor's degree required in Social Work or a related field; no nursing degrees accepted.
- Bilingual in Spanish and English is required.
- Must reside in Hillsborough County, FL, and within 2-3 miles of the 33624/33625 zip code.
- Candidates will travel approximately 75% of the time within the region to see members.
