Position Summary: The Community Health Coordinator will directly assist the Director of Community Health in engagement, implementation, and oversight of Community Care Plan’s targeted Health Risk Assessments, expanded benefit and healthy behavior programs. This position will ensure that Community Care Plan meets and/or exceeds contractual requirements for these programs.


Essential Duties and Responsibilities:

  1. Conducts outbound calls to targeted populations to complete health risk assessments in accordance with state contractual requirements and documents results in CCP’s varied software platforms.
  2. Assesses for social and clinical needs and when needs are identified, refers to appropriate CCP departments for follow-up.
  3. Reviews mail-in and electronically submitted Health Risk Assessment responses for Health-Related Social Needs (HRSN) and/or appropriateness for any Healthy Behavior programs, referring to C3 Care Management and Social Work when identified.
  4. Routinely tracks completed healthy behavior member requests and prepares incentive files in accordance with CCP’s established schedule.
  5. Assists Medical Economics with quarterly and ad hoc Healthy Behavior reporting per contractual requirements.
  6. Assesses Healthy Behavior utilization patterns and when needed makes recommendations for program changes to increase utilization.
  7. Supports member outreach and engagement efforts to increase awareness of Expanded Benefit programs.
  8. Monitors effectiveness of expanded benefit utilization via dashboards and other reporting tools as available.
  9. Works collaboratively with Corporate Finance to assess actual expanded benefit expenses for reporting purposes.
  10. Provides data and outcomes for reporting and presentation needs.
  11. Monitors and evaluates all programs for process improvement.
  12. Performs other related duties as assigned.

Qualifications:

  • Associate degree with at least one year of relevant experience.
  • Microsoft Office and internet software proficiency

  • Excellent verbal and written communication skills.
  • Correctly interpret data and guide decision making.
  • Ability to work independently.
  • Ability to meet deadlines.
  • Must be able to work in a fast-paced environment
  • Skilled in maintaining good rapport and collaborative working relationships with team members.

Work Schedule:

Community Care Plan is currently following a hybrid work schedule. The company reserves the right to change the work schedules based on the company needs.
