Position Summary:
The Outreach & Growth Coordinator supports the growth of our outpatient clinic by developing and maintaining referral relationships and facilitating the scheduling of Medicare outpatient evaluations. This is a non-clinical, relationship-based role measured by completed, billable patient care. The position plays a key role in connecting patients to therapy services while ensuring all outreach activities remain compliant with CMS regulations.
Key Responsibilities:
- Develop and maintain referral relationships with physician offices, Assisted Living Facilities (ALFs), Independent Living Facilities (ILFs), and community partners
- Conduct in-person and phone outreach to referral sources
- Coordinate scheduling of outpatient evaluations and follow up on referrals
- Track referral activity and sources accurately
- Prepare and submit weekly outreach and referral reports
Qualifications:
- Experience in healthcare outreach, business development, or referral coordination preferred
- Strong communication and relationship-building skills
- Organized, self-motivated, and goal-oriented
- Ability to track data, follow processes, and meet reporting deadlines
- Understanding of healthcare compliance preferred

Compliance & Scope of Role
- This role may not discuss Medicare benefits, eligibility, or guarantees of coverage
- May not offer anything of value to referral sources
- May not provide clinical advice
- All activities must comply with CMS regulations and the Federal Anti-Kickback Statute
Compensation Structure
Base Pay:
- Recoverable draw to satisfy minimum wage requirements
- Draw is offset against earned commissions
Commission Structure:
- $200 per patient after the first four (4) completed outpatient clinic visits
Commission Eligibility Requirements:
- Visits must be scheduled, completed, documented, and signed
- Visits must be billable to Medicare
- Commission is earned once per patient, after visit #4
Non-Qualifying Events:
- No-shows or cancellations
- Incomplete or undocumented visits
- Non-billable or denied services
Commission Timing:
- Commissions calculated bi-weekly
- Paid through payroll following verification
- Subject to reconciliation
