Payroll Title: EHS Disability Facilitator
Under the supervision and guidance of the Licensed Mental Health Professional and Disability, the Disability Facilitator work to implement high-quality comprehensive services to children with disabilities, that are developmentally, culturally, and linguistically appropriate (HSPPS 1302: Subpart F).
- Share culturally linguistically and developmentally appropriate, science-informed information regarding the needs of children with disabilities with staff and parents.
- Promptly address, with parental consent and involvement, with a referral to the local agency responsible for implementing IDEA for a formal evaluation to assess the child’s eligibility for services under IDEA as soon as possible. HSPPS 1302.33(3).
- In collaboration with the Family Workers/ERSEA unit, work to recruit and enroll children with suspected or identified disabilities, attending transition meetings with agencies who serve children with disabilities, to ensure the program meets the 10% mandated disability requirements (HSPPS 1302.14(b)(1)).
- Arrange for formal evaluation of a children who has been identified with a concern from a science-based screening assessment, teacher report or parental concern to refer the child to the appropriate agencies for evaluation as soon as the concerns has been identified (HSPPS 1302.61).
- Review and revise reports necessary for the delivery of disability related services for accuracy.
- Participate in meetings with the appropriate agency charged with evaluation of a child providing support and guidance to the families along with teachers and disabilities facilitators and other program staff (HSPPS 1302.61(b)).
- Ensure that services for children with disabilities are planned and delivered as required by their IEP or IFSP and that children are working towards the goals stipulated in those documents (HSPPS 1302.61(d))
- Collaborate with parents and the local agency responsible for implementing IDEA to ensure appropriate steps are taken when the child transitions from ESH to HS or from HS to the public schools (HSPPS 1302.61(c)(2)).
- Establish collaborative relationships and partnerships with community organizations and assist with the collection of In-Kind. (HSPPS 1302.53 a)
- Conduct general classroom visits to ensure documentation of children with disabilities is accurate. Monitor therapist attendance and activities.
- Understand the referral evaluation and service timelines required under IDEA and monitor the process from identification of a suspected concern to an evaluation and the construction of an IEP and the provision of services (HSPPS 1302.61(c)(3).
- Work with parents to obtain necessary parental consents and supporting documentation for the identification and provision of services with children with disabilities (suspected or identified) (HSPPS 1302.61(a)).
- Work with the agency responsible for IDEA to submit all relevant information including the physical exam, screening and assessment results, evaluation results, and consent forms – including exchange of information forms (HSPPS 1302.62).
- Work with parents to ensure children needs are met, and help parents become advocates for their children. Assist parents in accessing services and resources for their children (HSPPS 1302.62(a)).
- Maintain and protect confidential and respect for the privacy of others in accordance with HSPPS 1302.92(c)(iii)(iv).
- Meet regularly with staff to ensure that all internal referrals are being addressed and follow up is being conducted.
- Maintain up-to-date documentation in Child Plus of children’s progress and follow up.
- Administration has the authority to assign additional duties as seen fir for the benefit of the organization. HSPPS 1302.91
- Update Standard Operating Procedures Manual
Knowledge, Abilities and Skills
- Knowledge in Head Start/Early Head Start Performance Standards preferred
- Knowledge of the Individuals with Disabilities in Education Act (IDEA) and the local Part B and Part C providers within the state of Florida (Miami-Dade County) preferred
- Ability to work closely with agencies responsible for implementing IDEA services
- Ability to work closely with local child development specialists, including mental health professionals, to ensure that services are provided to children with special needs.
- Ability to multi-task and work under pressure
Qualifications/Requirements
- High School and some college coursework.
- Must be able to sit and stand for long periods of time
- Must be able to provide assistance with lifting 40 pounds
