The Post-Acute Program Manager will lead and oversee the coordination of post-acute care services to enhance Advanced Management USA's mission of improving patient outcomes, streamlining transitions, and reducing hospital readmissions. This role ensures quality, cost-effective care by collaborating with post-acute providers and implementing evidence-based strategies within the ACO REACH framework.
Key Responsibilities and Duties Post-Acute Care Manager
- Develop and implement post-acute care strategies that align with ACO REACH goals and Advanced Management USA’s mission.
- Monitor patient transitions across skilled nursing facilities (SNFs), home health, and long-term care.
- Ensure seamless care coordination by working with hospitals, providers, and case managers.
- Utilize data analytics to track readmission rates and post-discharge outcomes.
Provider Network Management
- Establish relationships with post-acute care partners, including SNFs, home health agencies, and rehab centers.
- Implement preferred provider networks and maintain quality standards for partner organizations.
- Conduct performance reviews and ensure compliance with CMS and ACO REACH regulations.
Patient Engagement & Health Equity
- Develop strategies to improve patient engagement in post-acute care, particularly for underserved populations.
- Address Social Determinants of Health (SDOH) to enhance health equity in post-acute transitions.
- Educate patients and caregivers about care pathways and available services.
Performance Monitoring & Compliance
- Track and report key quality and utilization metrics for post-acute services.
- Ensure compliance with CMS ACO REACH guidelines and state/federal regulations.
- Collaborate with finance and analytics teams to monitor the cost-effectiveness of post-acute interventions.
Care Coordination & Process Improvement
- Develop workflows and best practices for discharge planning and follow-up care.
- Implement evidence-based interventions to reduce preventable hospitalizations.
- Lead quality improvement initiatives in partnership with interdisciplinary teams.
Qualifications
Education & Experience
- Associate or Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or related field required
- Minimum of 2–5 years of administrative experience in a healthcare setting
- Project management experience preferred
- Demonstrated success managing teams and performance
- Experience in post-acute care settings (SNF, HHA, rehab, etc.) and using care coordination platforms (e.g., CarePort, Epic, or similar)
- Familiarity with ACOs, value-based care, or population health preferred
Skills & Competencies
- Proficiency in Microsoft Office (Excel, Outlook, Word)
- Excellent organizational and time-management skills
- Strong written and verbal communication abilities
- Ability to multitask and work collaboratively in a fast-paced environment
- Strong leadership and team management skills
- Excellent consulting and relationship management skills, with a proven ability to engage physicians and practice leaders.
Staring Salary:
- $65,000+ commensurate with experience
