Care Manager – Senior Home Companions – SW Florida

We are filling this role in our Punta Gorda Office and will be servicing the SW Florida market.


What you’ll do:

As a Care Manager, you’ll be the vital link between clients, families, and caregivers. You’ll build lasting relationships by:

· Understanding and tailoring home care solutions to each client’s unique needs.

· Coordinating caregivers with thoughtfulness and attention to detail.

· Communicating and documenting important caregiver and client feedback.

· Supporting our team to deliver care that not only meets but exceeds expectations.



Requirements:

  • Computer skills: Microsoft office, comfortable in learning client care management software systems
  • Must be available on weekends and/or on call as clients' needs come up
