Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for a Clinical Assistant/Scheduler to join our award-winning team.

Key Responsibilities

  • Receive, track, and ensure timely processing of all VA consults and referrals.
  • Conduct welcome calls with patients to introduce services and confirm readiness for care.
  • Follow up on Request for Services (RFS) forms to ensure timely return and completion.
  • Audit patient charts to confirm accuracy of physician information, orders, and required documents prior to admission.
  • Monitor and track VA patients admitted to hospitals, providing timely updates to the branch and care teams.
  • Facilitate resumption of care (ROC) by coordinating with branch staff and field sales representatives on pending discharges and orders.
  • Manage and track orders, including new referrals, ROC orders, additional orders, and hospital hold documentation.
  • Coordinate and monitor labs, IV therapy, and DME (durable medical equipment) requests to ensure continuity of care.
  • Communicate with clinicians and operations staff to resolve missing or incomplete documentation and support timely admissions.
  • Partner with field sales representatives assigned to VAMCs to identify and resolve referral barriers.
  • Provide regular updates to branches on patient status, expiring authorizations, and additional documentation needs.
  • Attend sales/marketing and VA-focused meetings as required to support collaboration and alignment.
  • Ensure that all activities comply with HIPAA, agency policies, and regulatory compliance requirements.
  • Support the Agency’s QAPI (Quality Assurance and Performance Improvement) initiatives through active participation and timely reporting.
  • Maintain acute awareness of patient rights, infection prevention standards, and reporting requirements for suspected abuse or neglect.

  • Strong interpersonal and relationship-building skills with the ability to collaborate across sales, operations, and clinical teams.
  • Excellent organizational and multi-tasking abilities, with high attention to detail.
  • Problem-solving mindset with the ability to identify gaps in referral and admission processes.
  • Proficient in written and oral communication.
  • Professional appearance, behavior, and commitment to service excellence.
  • EMR experience (Kantime and WellSky preferred).
