Community Outreach Representative

Reports to: Community Outreach Manager

FLSA Status: Non-Exempt

Position Summary

The Community Outreach Representative supports Nuvia Medical’s mission to transform the health of communities by transforming healthcare. Guided by our values — ICARE = Integrity, Compassion, Accountability, Respect, Excellence — this role engages directly with community members, patients, and partner organizations to increase awareness of Nuvia services, assist with health plan enrollment, and connect individuals to ACCESS Centers. The Community Outreach Representative serves as a frontline ambassador, ensuring respectful, compassionate interactions and driving access to care in underserved communities.

Essential Duties & ResponsibilitiesCommunity Engagement

  • Represent Nuvia Medical at health fairs, community events, and partner meetings.
  • Build relationships with community leaders, organizations, and healthcare providers.
  • Promote awareness of Nuvia’s mission, services, and ACCESS Centers.

Enrollment & Patient Support

  • Assist individuals with health plan applications and enrollment processes.
  • Provide education on available services and connect patients to resources.
  • Ensure a welcoming, supportive environment for all community members.

Access Center Support

  • Greet visitors, answer questions, and support front-line operations at ACCESS Centers.
  • Collaborate with Center Administrators and staff to ensure smooth processes.
  • Track and document outreach, applications, and follow-up interactions.

Compliance & Reporting

  • Meet established performance metrics for outreach contacts, applications, and enrollments.
  • Maintain accurate records of activities and submit required reports.
  • Adhere to HIPAA standards and all Nuvia Medical policies and procedures.

Qualifications

  • High school diploma or equivalent required; associate’s or bachelor’s degree in social work, public health, or related field preferred.
  • 1–2 years of experience in community outreach, customer service, healthcare access, or related field.
  • Bilingual (English/Spanish or English/Creole) strongly preferred.
  • Strong interpersonal and communication skills with the ability to build trust quickly.
  • Customer service orientation with empathy and problem-solving skills.
  • Ability to work evenings/weekends for events and travel locally as needed.
  • Basic proficiency with Microsoft Office and data entry systems.
  • Familiarity with healthcare systems, social services, or managed care organizations preferred.

Compensation

Minimum: $45,000

Midpoint: $50,000

Maximum: $55,000
