Private Duty Home Health Client Coordinator -

The Visiting Nurse Association of Florida is a 50 year old Service Driven organization that prides itself in Excellence. We seek Motivating professionals who will join our Committed team.

We are currently seeking an Client Coordinator with a can-do attitude and able to perform under pressure. Our ideal candidate will possess excellent communication skills while providing the highest level of customer service to internal and external customers. Every VNA of Florida employee is expected to be a valuable team player by taking the initiative to solve problems quickly and effectively. Our quality of work is paramount, so each applicant must have a strong eye for detail with a strong work ethic.

Main Responsibilities:

  • Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
  • Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
  • Customer Service and Concierge Skills that uses compassion, experience.
