Private Duty Home Health Client Coordinator -

We are currently seeking an Client Coordinator with a can-do attitude and able to perform under pressure. Our ideal candidate will possess excellent communication skills while providing the highest level of customer service to internal and external customers. Our quality of work is paramount, so each applicant must have a strong eye for detail with a strong work ethic.
Main Responsibilities:
- Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
- Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
- Customer Service and Concierge Skills that uses compassion, experience.
