Position Summary:
To assist patients and families with discharge planning. The Discharge Planner coordinates and implements post hospital care by evaluating and assessing the patients' needs, financial status/insurance coverage and community resources available. Discharge planning is coordinated with physicians, Nursing, Case Managers/Care Coordinators and the patients along with their families/significant others who will be involved in their post-hospital care.
Position Qualification:
Requires a mature individual, capable of being objective while empathetic and supportive of patients and family's needs. Must be able to communicate verbally with all levels of society. Must be able to read, write, and speak English fluently. Able to communicate effectively in Spanish as well. Ability to work with individuals of all ages, races, and religious backgrounds. Must possess good organizational skills and be able to remain calm in stressful situations. Basic computer skills required. Basic knowledge of community resources.
Required Licenses/Certifications:
Associate of Art or Associate in Science Degree preferred.
Must be high school graduate with three to five years' experience in an acute care setting and/or Social Work-related field preferred.

<li>Our colleagues begin each day energized and ready to make an impact. They wear their name badge with pride, greet teammates with a warm hello, and help ensure the sales floor is clean, organized, and prepared for customers. Before getting started, they review their sales goals, learn about new arrivals and top-selling items, and stay up to date on current trends and promotions - so they’re fully informed and ready to engage, educate, and sell with confidence.</li>

<li>On the floor, they focus on the customer - offering a warm welcome, making eye contact, and creating friendly, personal conversations to create customer relationships to bring the customers back.</li>

<li>Throughout the day, they maintain a shoppable space by recovering the floor and fitting rooms, updating signs and pricing, and supporting any needed merchandise moves.</li>
\n
<li>They always Finish Strong - sharing our Loyalty program benefits, making sure customers have all they need, and ending with a sincere thank-you, often walking the package around and using the customer’s name.&nbsp;</li>

Full-time colleagues are typically scheduled to work 30 or more hours per week, while part-time colleagues are typically scheduled 12-24 hours per week. All schedules contain weekend shifts as part of the requirements. Hours are dependent on business needs and colleague availability. Essential Physical Requirements You Will Perform
\n
<ul>\n
<li>This position requires talking, lifting, constant moving, standing, and reaching with arms and hands.</li>
\n
<li>Standing for at least two consecutive hours, lifting at least 25lbs. stooping, kneeling, crouching, and climbing ladders, reaching, including above eye level.</li>
\n
<li>Requires close vision, color vision, depth perception, and focus adjustment.</li>
\n
<li>Frequent use of computers and other technology, necessary to perform job functions, including handheld equipment, cash register and ability to process register transactions.</li>
