Key Responsibilities

<li>Provide consistent case management, meeting with patients at least once per week or more frequently as needed, and maintain accurate and detailed records in patients’ EMRs to support safety and recovery progress.</li>
<li>Monitor and document client behaviors, assess potential risks, and promptly report critical changes to clinical staff.</li>
<li>Prepare patients for post-discharge recovery by creating and implementing comprehensive discharge plans with appropriate continuing care and resources.</li>
<li>Maintain regular communication with families, referral sources, and external resources to facilitate patient support and ensure a cohesive care approach.</li>
<li>Foster a professional and empathetic environment by maintaining boundaries, demonstrating understanding of addiction and recovery processes, and applying evidence-based treatment methodologies.</li>
