As a Home Health LPN, you will:
- Provide skilled nursing care under RN supervision
- Deliver curative, rehabilitative, palliative, or preventive care as outlined in patient care plans
- Make informed decisions based on your education and experience
- Ensure accurate and timely patient documentation

Qualifications:
- Active LPN or LVN license (state-specific)
- Graduate of an accredited nursing program
- 1+ year of nursing experience (home health or medical/surgical preferred)
- Valid CPR certification
- Strong communication and organizational skills
- Reliable, insured transportation and a valid driver’s license
