A-Line Staffing is now hiring a Case Management Coordinator – Field (North Miami Area). This position offers a Monday–Friday schedule, remote flexibility, and meaningful field-based member support within Miami-Dade County.
Case Management Coordinator Compensation
- Competitive pay rate $34.13/hr
Case Management Coordinator Highlights
- Work-from-home administrative duties
- 75% field travel within Miami-Dade County
- Monday – Friday, 8:00 AM – 5:00 PM
- Remote training via Microsoft Teams
- Mileage reimbursement to and from member homes
- Candidates must reside in: North Miami, Aventura, Hialeah, or Miramar
- Bilingual English/Spanish REQUIRED (fluent speaking and writing)
Case Management Coordinator Responsibilities
- Conduct telephonic and face-to-face member assessments
- Manage and coordinate case management activities
- Determine medical necessity and appropriateness of services
- Develop proactive care plans to enhance member wellness
- Coordinate services including prior authorizations, PCP/specialist collaboration, medication review, and community resources
- Educate and empower members to promote compliance and patient advocacy
- Identify quality and compliance opportunities and initiate corrective action when needed
- Optimize total healthcare costs through effective care coordination
- Collaborate with providers and interdisciplinary teams
- Utilize Microsoft Office tools for documentation and reporting
Case Management Coordinator Requirements
- Bachelor’s degree required (non-nursing, non-social work degree)
- Case management experience required
- Must reside in North Miami, Aventura, Hialeah, or Miramar
- Fluent in English and Spanish (spoken and written)
- Microsoft Office proficiency (including Excel)
- Strong organization, multitasking, and communication skills
- High School Diploma or GED required
Preferred Qualifications
- Long-term care experience
- Experience working with medically complex populations
