JOB SUMMARY: Serves as the primary contact person who receives and triages calls from relative caregivers, self-referrals and community partners. Intake coordinator reviews referrals and assigns Kinship staff for program services.

ESSENTIAL FUNCTIONS:

  1. Facilitates the Kinship Services Referral Line, communicating effectively, appropriately and responds in a timely fashion to phone calls, faxes and correspondences.
  2. Screens referrals thoroughly within research and evaluation protocols for families that come into the Referral Line.
  3. Screens and reviews referrals with Kinship Director and Program Manager.
  4. Assists and supports Program Managers with contract assignments and Kinship staff.
  5. Prepares and maintains electronic database, forms and memos.
  6. Maintains precise and accurate records and statistics for Kinship Referral Line and Children’s Home Network programs as assigned.
  7. Communicates and educates community partners about Kinship Services;
  8. Collects data for research and evaluation for monitoring of program outcomes;
  9. Maintains effective relationships with supervisory staff, and community resource providers, emphasizing facilitation of referrals;
  10. Participates in organization’s continuous quality improvement efforts;
  11. Attends training and conferences as directed by Program Manager.
  12. Opens and closes cases in MyEvolv.
  13. Documents contacts with caregivers in FSFN.
  14. Performs other related duties as assigned.

MINIMUM QUALIFICATION REQUIREMENTS:

Education & Experience

High school graduate or equivalent from an accredited institution with 3 or more years administrative support experience in a Human Service Field. Bi-lingual, Spanish speaking required.

Licenses & Certifications:

  • Must possess and maintain a valid Florida driver’s license with no record of criminal driving offense or license suspension.
  • Must possess means of providing job related transportation and show proof of required insurance. Must be insurable under CHN’s current auto insurance policy.
  • Must be able to work flexible hours, including evenings and weekends.
  • Must successfully complete and maintain FDLE clearance, Federal background and state criminal background check and sexual predator screening.

COMPETENCIES & PROFESSIONAL DEVELOPMENT:

Annual Training Requirements: The following training topics are required annually: Sexual Harassment, Blood-born Pathogens, Workplace Accident Prevention and Reporting, Confidentiality and Privacy Practices, Diversity/Cultural Competency, Child Abuse and Neglect Mandated Reporter Statutes and Methods, Prevention of Violence in the Workplace, OSHA–related training, Crisis Prevention, and Program Goals.

Knowledge, Skills, Abilities & Competencies:

  • Ability to communicate effectively in speech and in writing.
  • Ability to use correct grammar, spelling and punctuation in written correspondence.
  • Ability to type and accurately enter data into computerized systems.
  • Proficiency in Microsoft Office (MS Excel, MS Word, MS Outlook).
  • Possess time management and prioritization skills.
  • Possess a positive attitude concerning all aspects of working in a challenging environment, including significant patience and respect for children and families who can become quite angry and demanding, sensitivity to cultural needs and willingness to serve as a positive member of a working team.
  • Bi-lingual in Spanish required.

Supervision Received:

Supervision is provided in the form of one-on-one supervision and regular team meetings. Specific guidelines and policies and procedures form the basis for performance monitoring.

ESSENTIAL PHYSICAL SKILLS:

  • Possess good hearing (with or without hearing devices)
  • Ability to use a personal computer and related office equipment. Frequent use of hands and fingers for typing
  • Ability to speak clearly and effectively
  • Ability to work within an office setting requiring keyboard skills and administrative activities of a sedentary nature
  • Ability to operate a motor vehicle
