Title: Care Manager
Openings: 1
Duration: 3-month contract – potential to extend
Hourly rate: $40-$48hr
Shift: Monday-Friday (8:00 AM-5:00 PM)
Interview Process: Onsite, 1 and done.
Must Haves:
- Registered Nurse license (FL) and BLS certification
- 2 years of experience as a Care Manager
- Ability to interact and verbally communicate effectively with patients and all levels of personnel in a professional manner.
Nice to have:
- Medicare Advantaged Care experience
- IV certified
Day to Day:
- A client in the Deltona, FL area is looking for a Care Manager to join their team. The position will assist with reduction in overall admissions, readmissions, and ER visits. The CM is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. The CM assists in developing patient empowerment by acting as an educator, resource, and advocates for patients and their families to ensure a maximum quality of life. The CM interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, social workers, and other educators.
Additional responsibilities:
• Meets with patients, patients’ families and caregivers as needed to discuss care and treatment plan as delineated in Transitions of Care program.
• Identifies and assists with the follow-up of high-risk patients in acute care settings, skilled nursing facilities, custodial and ambulatory settings.
• Uses protocols and pathways in line with established disease management and care management programs and approved by medical management to optimize clinical outcomes.
• Oversees provisions for discharge from facilities including follow-up appointments, home health, social services, transportation, etc., to maintain continuity of care.
• Maintains effective communication with the physicians, disease management, hospitalists, extended care facilities, patients, and families.
• Demonstrates a thorough understanding of the cost consequences resulting from Care Management decisions through utilization of reports and systems such as Health Plan Benefits, utilization of metrics and CM reports.
• Participates in daily huddles and Patient Care Coordination (PCC) meetings
