Hours: Monday–Friday, 8:00 AM – 5:00 PM
MUST HAVES:
- Bachelor’s degree
- 2+ years of related experience
- Valid state driver’s license
- Strong verbal/written communication and interpersonal skills
- Ability to use various electronic information systems
PLUSSES:
- Master’s degree
- Experience with Medicare & Medicaid recipients
- Familiarity with electronic case note documentation
- Experience in health promotion, coaching, and wellness
- Knowledge of community health and social service agencies
JOB DESCRIPTION:
The Care Coach 1 is responsible for evaluating members’ health needs and supporting their journey toward improved wellness. This includes helping members and their families connect with appropriate resources, identifying and overcoming obstacles to effective care, and tracking progress toward health goals. The role involves developing and managing care plans using assessments, data insights, and member conversations. Work is performed in alignment with professional standards and established procedures, with room for some interpretation.
