Per Diem Physical Therapist (Outpatient)

Setting: Outpatient Rehabilitation
Susanna Wesley Healthcare Center is a trusted provider of rehabilitation and skilled nursing services in Hialeah, Florida. Our outpatient therapy department is dedicated to helping patients regain function, improve mobility, and enhance quality of life through personalized, evidence-based physical therapy care.
We’re seeking a compassionate and motivated Per Diem Physical Therapist to join our outpatient rehab team. This is an excellent opportunity for therapists who enjoy flexible scheduling and a collaborative, patient-centered work environment.
Responsibilities
- Evaluate, plan, and implement individualized physical therapy treatment programs for patients in the outpatient setting.
- Use a variety of techniques and modalities to improve strength, mobility, balance, and function.
- Educate patients and caregivers on exercises, safety, and home programs.
- Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards.
- Collaborate with an interdisciplinary team including occupational and speech therapists, nurses, and physicians to ensure optimal patient outcomes.
- Maintain compliance with all applicable state, federal, and professional regulations.
Qualifications
- Licensed Physical Therapist in the State of Florida (required).
- Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
- Prior outpatient or rehabilitation experience preferred, but new grads are welcome to apply.
- Excellent communication, time management, and documentation skills.
- Ability to work independently and as part of a team.
Schedule & Compensation
- Per diem/PRN schedule – flexible hours based on patient load and availability.
