Community hospital facility looking to bring on Director Case Management! Sign On Bonus, Bonus Incentive Plan and Full Relocation!

Overall responsibility for managing and coordinating department activities. Ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the department's patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization.

• Ideal candidate: RN active license, 3+ years of acute Hospital Case Management & 2+ years of leadership experience. Would be open to seeing a strong social worker, with active LCSW at a larger hospital & strong leadership skills.
• Does this position need to have recent acute care experience (Within the last year)? Yes
• Who does this position report to? Division VP of Case Management, W FL Division
• How many total beds are in the unit? 200+ beds
• Trauma level: Level 1 Trauma
• Volume: around 989 discharges a month
• How many FTEs will the candidate be overseeing? 15 FTE’s

Qualifications:

• Bachelor's degree, required
• Master's degree in Nursing, Health Administration, or Business Administration, preferred
• Either Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), required
• 3+ years’ experience in overall acute care Hospital Case Management, REQUIRED
• 2+ years’ experience in case management leadership in acute care REQUIRED
