About the job:
The Occupational Therapist (OT) will provide ongoing therapy to residents in a timely manner per physician’s orders. Communicate progress within the facility of resident progress. The Occupational Therapist evaluates and treats patients, communicates with families, physicians and other health team members and maintains appropriate documentation of services including MDS responsibilities complying with State and Federal regulations. Participates in on-going facility meetings and conducts in-service and training for facility staff on ongoing basis in restorative and maintenance techniques.
QUALIFICATIONS:
- Graduate of an occupational therapy curriculum.
- Current Florida State Occupational Therapist License
- A minimum of one year’s experience working with a frail or elderly population required
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
- Ability to pass drug, criminal and employment screenings.
ESSENTIAL FUNCTIONS:
- Conducts an in-person initial comprehensive occupational therapy assessment on all participants upon enrollment and coordinates with the Interdisciplinary Team on a Plan of Care for each participant. And Conducts an in-person re-assessment minimally every six (6) months or as needed.
- Provides evaluation/treatment on acute, restorative and maintenance levels to participants.
- Functions as a member of the Interdisciplinary Team.
- Administers treatment program per physician instructions.
- Carries out accepted principles and practices of occupational therapy for prevention of functional decline, sensory and cognitive limitations, increase or maintenance of strength and endurance, and provides treatment to increase self-care skills.
- Constructs or provides orthotic and adaptive equipment as needed.
- Conducts assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety.
- Assists as liaison with nursing home and assisted living staff regarding occupational therapy needs of participants who are their residents.
- Evaluates charts and reports on participant’s progress, documents occupational therapy notes in the participant’s medical record. Fulfills charting and reporting requirements.
- Directs, supervises, performs initial and annual competencies on, and evaluates the performance of the Certified Occupational Therapy Assistant.
