Education

  • Graduate of an accredited school of professional nursing, BSN required

Experience / Qualifications

  • Current RN licensure in the state of Florida.
  • BLS certification through the American Heart Association for Healthcare Providers.
  • ACLS certification through the American Heart Association for Healthcare Providers.
  • NIHSS training can be obtained through various organizations and learning systems that offer courses from the American Heart Association.
  • A minimum of 2 years of Cardiovascular care hospital experience; previous experience working with structural cardiology required.
  • Clinical expert in Cardiovascular nursing practice and in the delivery of evidence-based nursing interventions
  • Demonstrates critical thinking and expansive knowledge of Cardiovascular care skills, in addition to care of the patients undergoing structural heart procedures.
  • Provides a positive influence to improve outcomes and satisfaction of the patients, families, nurses, and the healthcare team.
  • Ability to collaborate effectively as a team leader in a dynamic health care environment, with the ability to work autonomous.
  • Demonstrate expertise in caring for patients across the adult age continuum.

Position Summary

The Structural Heart Coordinator (SHC) is responsible for daily operations of the structural heart program. They work with stakeholders in the development, evaluation and implementation of evidenced based practice processes that drive quality outcomes with various structural heart procedures. Specific responsibilities include, but not limited to:

  • Triage and manage new patient referrals into the program; obtain patient records.
  • Coordination of patient evaluation and screening processes
  • Ensure completeness of Heart Team evaluation
  • Communicate patient goals and social needs to the Heart Team
  • Serve as primary contact for outpatients.
  • Clinical support for testing, scheduling, and coordination
  • Coordinate patient tests with local providers
  • Verify procedural readiness prior to procedural scheduling.
  • Serve as a contact for referring physicians.
  • Oversight of the TAVR, TEER, LAAO, and other complex cardiac procedures
  • The SHC is responsible for collaborating with procedural areas, inpatient units, anesthesia, and appropriate personnel in scheduling of structural heat procedures.
  • Educates patient prior to on-site evaluation.
  • Coordinate industry procedure support; transmit imaging to industry vendors.
  • Complete orders for diagnostic testing
  • Oversee throughput for all structural heart patients
  • Coordinated agenda for the Structural Heart Multidisciplinary Meeting to evaluate patients for surgical intervention or referral for additional resources and follow up imaging.
  • Coordinate with providers on peri-procedure medication changes coordinate care and communication during procedure
  • Perform quality of life (KCCQ) evaluation
  • Assess patient stated goals for care.
  • Send records to referring and local providers, in addition to patients.
  • Schedule follow-up appointments
  • Assist with data submittal to the required registries, presents registry data and outcomes to meetings/committees as directed.
  • Oversees education and training of new structural procedures
  • Trains and educates new structural heart team members/ units/departments on structural heart topics as appropriate
  • Manage patient and family education.
  • Coordinates activities among defined clinical areas and facilitates an atmosphere of interactive management and collegial relationships.
  • Coordination of inter-departmental processes and communication including the ability to design and align resources to support program development and growth.
  • This position utilizes appropriate clinical competency to determine cardiovascular disease program follow-up and continuity of care in collaboration with the health care team.
  • Performs other duties as assigned.

Physical Requirements

Requires sitting for long periods of time, also stooping, bending, standing, and stretching. Must have manual dexterity for typing and computer data entry. Able to work under a stressful environment, work independently, capable of making sound decisions, be detail oriented, alert, and self-motivated. Ability to push/pull up to 40lbs., carry and lift up to 20lbs.

Threshold Requirements

  • These threshold requirements are required and completed yearly basis
    • Annual Joint Commission mandatory education requirements, in-service and health requirements including attendance at new employee orientation
    • TB/PPD Surveillance Program
    • Maintenance of required professional licensing and/or certification(s).
