We are seeking a detail-oriented and proactive Patient Intake Coordinator to join our Home Health team! This role is essential in ensuring seamless patient onboarding, efficient documentation management, and strong communication with insurers and caregivers.
Responsibilities:

Oversee patient intake process, verifying documentation and insurance eligibility

Accurately enter and maintain patient data within our electronic health records (EHR) system

Serve as the primary liaison between patients, caregivers, physicians, and insurance providers to facilitate a smooth transition of care

Coordinate and obtain necessary authorizations from insurance companies

Ensure compliance with Medicare/Medicaid requirements and Home Health regulations

Support clinical staff with administrative tasks to maintain operational efficiency

Maintain strict confidentiality and professionalism when handling patient information
Qualifications:

Previous experience in healthcare intake, administration (Home Health preferred)

Knowledge of Medicare, Medicaid, and private insurance authorization processes

Strong organizational and multitasking abilities to excel in a fast-paced environment

Proficiency in healthcare software (e.g., Availity, Homecare Homebase, or other EHR systems)
