We are seeking a compassionate and motivated Home Health Physical Therapy Assistant (PTA) to provide high-quality, patient-centered care in the comfort of patients’ homes. Under the supervision of a licensed Physical Therapist, the PTA will assist patients in improving mobility, strength, balance, and overall functional independence.

Responsibilities

• Provide physical therapy treatments as outlined in the Physical Therapist’s plan of care
• Assist patients with therapeutic exercises, gait training, balance activities, and functional mobility
• Educate patients and caregivers on home exercise programs and safety techniques
• Monitor and document patient progress, responses to treatment, and any changes in condition
• Communicate effectively with supervising Physical Therapist and interdisciplinary care team
• Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards
• Ensure patient safety and follow infection control and HIPAA guidelines
• Travel to patient homes within assigned service area

Qualifications

• Associate degree from an accredited Physical Therapy Assistant program
• Current PTA license in good standing (state-specific)
• Valid driver’s license, reliable transportation, and auto insurance
• Current CPR certification
• Strong communication and interpersonal skills
• Ability to work independently in a home-based setting
• Home health experience preferred, but not required
