Job Summary:
Responsible for supporting the VA Program Manager in the performance of both fiscal and program activities, focusing on the coordination and facilitation of services to the veterans being served through the Veterans Directed Home and Community Based Services Program (VD-HCBS).
Duties and Responsibilities:
- Provide training to Veterans and/or their Representatives.
- Conduct quarterly home visits for assigned Veterans.
- Complete intake of new VD-HCBS referrals, including submission of enrollment paperwork to Financial Management Service (FMS) and Department of Veteran Affairs (VA).
- Track and document correspondence for VD-HCBS referrals and maintain Veteran files.
- Review Veteran’s timesheets and enter, correct, or update data as needed in electronic database.
- Create and review Veterans’ budget plans to ensure accurate reporting.
- Assess Veteran needs and review that services provided are appropriate.
- Develop improvement plans or corrective action plans, as necessary.
- Communicate with VA Program Manager and FMS upon learning of a veteran's institutional placement (i.e., placement in a hospital, nursing home or rehab center).
- Work closely with VA Program Manager for all new enrollments, change in veteran status, and issue resolution.
- Communicate with the VA Program Manager regarding any veteran issues, approval of veteran paperwork and any changes in status.
- Communicate with the FMS regarding enrollments, budget plan revisions, and other veteran/fiscal agent related matters.
- Provide ad hoc reports as needed.
- Update and submit the VD-HCBS Monthly Update Spreadsheet by deadlines.
- Complete background check for all employees that the Veteran decides to hire.
- Create a Summary page for every Veteran and update it accordingly.
- Provide home visit/phone call spreadsheet with detailed notes to VA manager by the 10th of every month for the prior month.
- Complete mileage in NetSuite and submit with a back up detailed sheet by the 7th of every month for the prior month.
- Participate in meetings/conference calls with the local and national VA.
- Complies with all security and confidentiality regulations.
- Any other duties as assigned by VA Program Manager, Controller, COO.

Education:
Associates or Bachelor's (preferred) degree. 2+ years' experience in case management (preferred). Equivalent combination of education and experience.

- Team oriented with willingness to collaborate.
- Strong listening and information gathering skills.
- Ability to work independently and assist with multiple projects, priorities, and locations.
- Working knowledge of federal and state funded programs.
- Proficient in the use of Microsoft Word and Excel.
- Good written and oral communication skills.
- Ability to meet deadlines.
- Ability to drive to a variety of locations.
Special Requirements: Must pass DOEA Level II criminal background screening; must sign Medicaid Attestation Payroll Form per Department of Elder Affairs/AAAPP requirements. Must sign SMMCLTC Program – Prohibited Activities. For this position, driving is as a regular component of the job, you will be required to have a non-expired Florida driver’s license for a Motor Vehicle Report (MVR) and must provide a copy of a valid driver’s insurance card.
