A-Line Staffing is now hiring a Field Case Manager in Naples, FL.
The Field Case Manager position is a full-time, field-based opportunity. We’re looking for a compassionate and experienced care professional to join our team! This role is primarily in the field (80–90%) within Naples, FL region.
Field Case Manager Compensation
- Pay Rate: $30.00 per hour starting
- FTE Conversion Range: $68,000–$70,000 annually
· Benefits are available to full-time employees after 90 days of employment
· A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Field Case Manager – Highlights
- This position is a contract assignment with intent to convert!
- MUST PASS TYPING TEST OF 40+ WPM
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Location: Remote/Field-Based – Must reside in Florida’s Region 11
- Travel: 80–90% fieldwork; must have a valid driver’s license
Field Case Manager – Responsibilities
- Conduct in-home or community-based assessments and care planning for members with long-term care needs
- Develop and monitor individualized service plans for elderly and/or medically complex members
- Coordinate services with healthcare providers, caregivers, and other agencies to ensure timely care delivery
- Support members and families with accessing services like housing, employment assistance, or home care
- Provide education to members and caregivers regarding benefits, service availability, and care coordination
- Maintain accurate documentation; all entries must be completed within 24 hours of contact
- Complete monthly and quarterly follow-ups, primarily through in-person visits
- Help resolve complaints and support appeals
- Attend virtual training sessions and shadow field-based case managers during onboarding
Field Case Manager – Requirements
- Education: Bachelor’s degree in Social Work, Psychology, Nursing, or a related human services field
- Experience:
- At least 2 years of targeted or field-based case management
- Long-term care or home health experience preferred
- Comfortable managing caseloads of 60–70 members
- Technical Skills:
- Strong Microsoft Office and EMR experience
- Minimum typing speed of 40 WPM (include results with resume)
- Soft Skills:
- Excellent verbal and written communication
- Time management and documentation accuracy
- Adaptable, self-driven, and a strong team player
