Intake Specialist (Salaried, Sales-Oriented, Flexible)
Location: Tampa, Florida (Coworking space provided: e.g., Hyde House or Industrious)

Position Summary
United Heritage Group (UHG) is seeking a results-driven Intake Specialist with a strong sales background and “closer” mindset to support our California-based Home Health and Hospice division. Based in Tampa, FL, you will manage the full intake cycle—from referral receipt to patient admission—ensuring fast turnaround, clinical eligibility, and seamless onboarding of new patients.
This role is salaried with flexible hours, requiring availability during off-hours and weekends to monitor and convert incoming referrals. If you thrive in fast-paced, high-responsibility environments and have a passion for healthcare coordination and patient care, this role is for you.
Schedule & Availability

Regular Hours: 10:00 AM – 6:30 PM EST (8 hours/day with a 30-minute unpaid lunch)

Time Zone Alignment: Matches California office hours (7:00 AM – 3:30 PM PST)

Off-Hours Expectation: Must monitor referrals during evenings and weekends as needed
Key Responsibilities
Referral Management
-
Monitor intake portals and emails (Ensocare, Aiden, Aida, Careport, fax)
-
Categorize referrals by insurance type and service area
-
Initiate contact with referral sources to confirm receipt and gather any missing documentation

Perform real-time eligibility checks for:

Medicare (NGS Connex)
-
Private Insurances (UHC, Anthem Blue Cross, Tricare West, Alignment)

Sales & Patient Engagement
-
Call patients for introduction and onboarding using approved intake script

Schedule Start of Care (SOC) visit and assign RN
-
Follow up until admission is confirmed
-
Act as a closer, converting eligible referrals into admissions quickly
Coordination & EMR Entry

Upload patient profiles, history & physical, and intake documents into Axxess
-
Send clinicians complete admission packets
-
Monitor SOC completion and follow-up for LUPA compliance
-
Track and update GroupMe, Admissions Report (Pending/Non-Admit), and internal communications
Off-Hours & Weekend Expectations

Monitor referrals on Weekends, evenings, and holidays

Be reachable and responsive to urgent admissions or last-minute SOC requests

Coordinate across time zones and provide flexibility for high-priority referrals

Required Qualifications

2+ years in sales, intake, or healthcare coordination roles

Experience with Medicare and private insurance eligibility

Strong closing skills and follow-through

Tech-savvy: Able to learn portals and EMR systems quickly (preferably Axxess)

Excellent written/verbal communication and problem-solving skills

Must reside in Tampa, FL and work California time (PST)
Preferred:

Background in home health, hospice, healthcare intake, or insurance verification

Experience with Axxess EMR
