Job Title: Nurse Case Manager
Location: Miami-Dade County
Duration: 04 Months
Pay rate: $35-$38 on W2
Job Description:
Job Title: RN Case Manager focused on nursing facility rebalancing and community transitions
Location: Must reside in Miami-Dade County; covers Miami-Dade & Florida Keys
Travel: Up to 75% local travel
Schedule: Monday–Friday, 8:00 AM–5:00 PM EST (flexible)
Key Responsibilities:
- Complete 10+ NF member assessments weekly
- Transition members from nursing facilities to the community
- Conduct in-person facility and home assessments
- Complete Significant Change Visits and Plans of Care
- Collaborate with case managers, facilities, and social services
Requirements:
- Active Florida RN license
- 2+ years clinical experience
- Bilingual Spanish/English required
- Case management or transition-of-care experience preferred
