Patient Home Care Coordinator
Duties:
- Answer incoming calls and provide assistance to patients and healthcare providers
- Schedule appointments and coordinate patient care activities
- Maintain accurate and up-to-date patient records in the electronic health record system
- Collaborate with healthcare professionals to ensure seamless care transitions
- Provide information and resources to patients and their families regarding healthcare services and community resources
- Follow up with patients to ensure they are receiving appropriate care and support
- Assist with insurance verification and authorization processes
- Coordinate referrals to specialists and other healthcare providers
Skills:
- Excellent communication skills, both verbal and written
- Strong organizational skills and attention to detail
- Ability to multitask and prioritize tasks effectively
- Proficient in using phone systems and electronic health record systems
- Knowledge of medical terminology and healthcare processes
- Ability to work well in a team environment
- Compassionate and empathetic towards patients' needs
- English and Spanish required
