Watercrest Palm Beach Gardens is looking for a Wellness Nurse (LPN/RN) to be selected to join an exclusive team! As an high-end Memory Care Nurse you will be responsible for overseeing the daily clinical operations of our beautiful first class Concierge Memory Care Residence. This role provides hands-on nursing support, medication oversight, associate direction, and coordination with families, physicians, and the interdisciplinary team. This role promotes a calm, organized, and person-centered environment consistent with dementia-care best practices and community standards.


ESSENTIAL JOB FUNCTIONS:

  1. Clinical Oversight & Resident Care
  • Conduct daily resident rounds to assess physical, emotional, and cognitive status.
  • Monitor changes in condition and initiate appropriate interventions, follow-up, and notifications.
  • Manage chronic conditions in alignment with care plans and physician orders.
  • Provide direct nursing care as permitted by licensure and community scope as needed.
  • Ensure documentation is accurate, timely, and consistent with state regulations.
  1. Medication Management
  • Preform medication administration for the Concierge Memory Care Residence.
  • Review MARs, orders, pharmacy updates, and ensure accuracy and compliance.
  • Participate in medication audits, reorders, and quality checks.
  1. Leadership & Associate Support
  • Serve as the on-duty clinical leader for the Concierge Memory Care associates.
  • Provide coaching, guidance, and on-the-spot training to Personal Assistants.
  • Support new employee onboarding and competency validation.
  • Promote teamwork, communication, and dementia-informed interactions among associates.
  1. Care Planning & Coordination
  • Participate in resident assessments, service plan updates, and interdisciplinary care reviews.
  • Provide clinical insight related to ADLs, risk factors, behavioral changes, and progression of dementia.
  • Ensure care plans reflect resident needs, preferences, and current clinical status.
  • Coordinate with the Memory Care Resident Wellness Director, physicians, therapy providers, hospice, and outside partners.
  1. Dementia-Informed Behavioral Support
  • Utilize person-centered approaches to manage behavioral expressions such as anxiety, agitation, or exit-seeking.
  • Coach staff on redirection, validation techniques, and avoiding triggers.
  • Collaborate with families to understand personal history and comfort strategies.
  • Document and report behavioral changes promptly.
  1. Safety, Compliance & Quality Assurance
  • Maintain compliance with all state regulations, community policies, and assisted living standards.
  • Ensure infection control practices are consistently followed.
  • Monitor for fall risks, skin integrity concerns, hydration issues, and other clinical priorities.
  • Participate in incident investigations.
  • Maintain accurate logs, audits, and clinical reports.
  1. Family & Resident Communication
  • Serve as a clinical point of contact for families regarding resident health updates.
  • Communicate changes in condition, new orders, or concerns promptly and professionally.
  • Support families during transitions and care decisions with empathy and clarity.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Strong clinical judgment and assessment skills.
  • Knowledge of dementia care, behavioral management, and person-centered approaches.
  • Ability to lead, coach, and motivate staff in a supportive manner.
  • Strong organizational skills and attention to detail.
  • Effective communication skills with residents, families, and team members.
  • Ability to remain calm, patient, and solutions-focused in a fast-paced environment.


EDUCATION REQUIREMENTS:

  • Must pass skills review/ competency
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with active state licensure.
  • Dementia or Alzheimer’s specialty training preferred.

EXPERIENCE PREFERENCES:

  • Minimum One (1) year in the senior living environment
  • Experience supervising care staff or leading clinical teams is preferred.

PHYSICAL REQUIREMENTS:

• Able to stand or walk most of the day.
• Able to focus despite frequent interruptions.
• Able to work under stress and during emergencies.
• Able to sit, stand, walk, reach, pull, push, and grasp as needed.
• Able to occasionally lift or carry up to 50 lbs.

OCCUPATIONAL EXPOSURE / PERSONAL PROTECTIVE EQUIPMENT:

  • Works throughout the community and may be exposed to dust, chemicals, infectious waste, and odors.
  • Uses required PPE (gloves, masks, non-slip shoes, infection control supplies) to prevent burns, falls, and infection.
  • Follows all safety policies and procedures.
