Responsibilities:
- Assess patients' needs and develop personalized treatment plans in the home setting
- Provide therapy to improve daily living skills and overall functions
- Collaborate with caregivers and families to support patient goals
- Educate patients and families on adaptive strategies and home modifications
- Monitor and document patient progress and outcomes regularly
- Implement interventions tailored to the home environment
- Participate in care coordination with healthcare teams and community resources
Qualifications:
- Master's degree in Occupational Therapy
- Active Florida Occupational Therapy Licensure
- Clean driving record
- Ability to work effectively in a Home Health setting
- Strong communication, time management, and clinical decision-making skills
Preferred Skills and Qualifications
- One or more years of experience in a Home Health setting
- Familiarity with electronic documentation systems
- Demonstrated commitment to professional growth and patient-centered care
