Exempt/Non-Exempt Position: Non- Exempt, Per Diem hours not to exceed over 19 hours a week. Weekdays and Weekend Days based on the needs of the team, department, and clients.
Minimum Qualifications:
Education: Nursing degree from accredited program
Licensure: Licensed Practical Nurse in the State of Florida.
Experience: Experience with individuals diagnosed with a severe and persistent mental illness preferred.
Must be 21 years of age.
Negative Drug Screening prior to hire and throughout employment.
Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver’s License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).
Working Conditions:
- Required to drive own car and/or company vehicle. Must meet established agency insurance requirements and approved driver status throughout the course of employment
- FACT office, as well as in the community where most contacts occur. Exposure to extreme heat and humidity during the summer months.
- On-call covering nighttime and weekend hours as requested.

Job Duties:
In collaboration with the team psychiatrist, coordinates, schedules, and administers medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinates psychiatric treatment with medical treatment.

- Under the direction of the team psychiatrists and in collaboration with other RN’s on the team, organizes and manages the system of getting medication to clients and ensures that the Medication Delivery Record is maintained for each client.
- Under the direction of the team psychiatrist, and in collaboration with other RN’s, records medication prescriptions on each client’s medication record and writes the orders in the Medication Record. Ensures that the records are accurate and up-to-date.
- In collaboration with other RN’s, orders medications from the pharmacy for each client as needed, arranges for delivery or pickup and ensures that all clients’ medications are available and packaged so that the client can take the medication as prescribed.
- For an assigned group of clients, completes a nursing assessment and collaborates with the case coordinator and other treatment team members on the client’s treatment plan, ensuring that medication administration and education are a part of the plan.
- For an assigned group of clients, uses solution-oriented strategies of intervention and principles of recovery that are consistent with best practice principles. Uses respectful and competency-based words and actions with clients and families while providing direct clinical services related to treatment plan goals that focus on medication administration, education regarding medication side effects, symptoms of illness and other treatment plan goals.
- As case coordinator for a group of assigned clients ensures that all continuing documentation is completed in the required time frame; including status letters to the court, outcomes, FARS, treatment plan addendums and reviews and obtaining needed releases of information for client hospitalizations, doctor’s visits, etc.
- Completes all other documentation correctly and in the required time frame including progress notes with objective to treatment plan clearly stated, and other documentation as requested by the team leader or needed to ensure all client records are complete.
- As part of a multi-disciplinary team, cooperates and participates fully with the FACT Team on-call response system. Provides on-call crisis intervention covering nighttime and weekend hours as requested. Demonstrates flexibility in support of the overall mission of the FACT Team Program in the delivery of the on-call services.
- Ensures that all assessments, treatment plans and progress notes are completed with accuracy, attention to detail and demonstrate excellent clinical skills and judgment and involve the client and his/her family input, if appropriate.
- As case coordinator assists clients in developing realistic, effective and measurable treatment plan goals and objectives. Establishes and prioritizes behaviorally descriptive, realistic desired changes with clients, and develops a realistic plan for their accomplishment with appropriate time frames.
- Participates in daily staff organizational meetings and treatment planning review meetings. Discusses cases appropriately at staffings. When not able to be present at a daily meeting, calls in with specific review of client contacts over the previous 24-hour period.
- Coordinates with client’s family members, other FACT staff and community supports to ensure client needs are met and all involved with client’s treatment work together.
