Physical Therapist (Full-Time / Per Diem)
 Lake Placid Health &amp; Rehabilitation Center – Lake Placid, FL In-House Subacute Rehabilitation
Lake Placid Health &amp; Rehabilitation Center is seeking a licensed Physical Therapist for full-time and per diem positions in our in-house subacute rehab program.
Responsibilities:
<ul>
<li>Evaluate and treat residents following illness, injury, or surgery</li>
<li>Develop and implement individualized treatment plans</li>
<li>Document care and patient progress per facility and regulatory standards</li>
<li>Collaborate with the interdisciplinary care team</li>
</ul>
Qualifications:
<ul>
<li>Graduate of an accredited Physical Therapy program</li>
<li>Current Florida PT license</li>
<li>Subacute rehab or skilled nursing experience preferred (new grads welcome)</li>
<li>Strong communication and teamwork skills</li>
</ul>
