Clinical Pharmacist - Medication Therapy Management (MTM)

** Must hold a current pharmaceutical license in the state of Florida **

Our client is is seeking a pharmacist with MTM (Medication Therapy Management) experience to join their Product Team for an initial 3 month project with a very real chance to extend. The pharmacist's responsibilities will include making outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. Work will be performed through a third-party, web-based application.

Must Have:

• At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)
• Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.
• Understanding of pharmaceutical and medical vocabulary
• Clinical experience in medication review.
• Excellent verbal and written communication skills
• Experience using salesforce preferred
• Must be tech savvy
• Self-directed and organized
• Discrete/ability to maintain confidentiality
• Detail-oriented
• Sense of urgency

Job Responsibilities:

• High volume of outbound phone calls. (100 calls per day/warm contacts)
• Completing comprehensive medication reviews.
• Conducting medication therapy management services.
