Key Responsibilities Patient-Centered Care & Clinical Excellence
- Conduct comprehensive initial assessments to determine individualized home care needs.
- Develop, implement, and continuously evolve patient-centered care plans.
- Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions.
- Administer treatments, medications, and interventions as prescribed—bringing hospital-quality care to the home.
- Empower patients and families through health education and hands-on guidance.
- Support proactive discharge planning to ensure seamless care transitions.
- Serve as Case Manager when designated, coordinating holistic care and ensuring continuity.
Communication & Collaboration
- Maintain clear, timely, and accurate clinical documentation.
- Collaborate closely with physicians, care coordinators, and interdisciplinary teams.
- Serve as a vital liaison between the patient, family, and healthcare providers.
- Participate in case conferences and advocate for optimal patient outcomes Additional Duties
- Participate in on-call rotation for 24/7 care continuity.
- Coordinate necessary equipment, services, and resources for effective home treatment.
- Supervise and evaluate home health aides every 14 days, ensuring top-tier care delivery.
Qualifications & Must-Haves
- Graduate of an accredited nursing program (Associate or BSN preferred).
- Active RN license in the state, current CPR certification, and valid driver's license.
- 1–2 years of acute care experience; home health care experience preferred.
- Strong communication, critical thinking, and clinical documentation skills.
- Capable of lifting, positioning, and transferring patients—hands-on and ready for action.
