Job Description

Job Title: Patient Coordinator


The Wellness Patient Success Coordinator serves as the primary point of contact for patients, guiding them through their care journey in hormone optimization, sexual health, and weight loss programs. This role is responsible for fostering strong patient relationships, ensuring seamless communication, managing prescription order refills, and driving business growth by closing sales and renewing memberships.


Key Responsibilities


  • Patient Communication & Guidance
  • Serve as the main liaison for patients, providing compassionate support and clear communication throughout their care journey.
  • Educate patients on treatment plans, services, and membership benefits.
  • Proactively check in with patients to monitor progress and address concerns.


  • Care Coordination
  • Guide patients through scheduling, follow-up appointments, and treatment milestones.
  • Collaborate with clinical staff to ensure a smooth patient experience.


  • Prescription Management
  • Coordinate and manage prescription order refills, ensuring timely and accurate processing.
  • Liaise with pharmacies and providers to resolve any prescription issues.


  • Sales & Membership Management
  • Present and explain membership options, treatment packages, and wellness services.
  • Close new patient enrollments and renew existing memberships.
  • Meet or exceed monthly sales and retention targets.


  • Administrative Support
  • Maintain accurate patient records and documentation in the practice management system.
  • Assist with front desk duties as needed, including answering phones and greeting patients.


Qualifications

  • Experience in patient coordination, medical office administration, or wellness sales preferred.
  • Strong interpersonal and communication skills.
  • Ability to multitask and manage competing priorities in a fast-paced environment.
  • Sales acumen and comfort with membership-based business models.
  • Knowledge of hormone optimization, sexual health, or weight loss programs is a plus.
