Psychiatric Mental Health Nurse Practitioner (PMHNP)
Locations: Englewood, Port Charlotte, and Sarasota, Florida
Job Summary
We are seeking a compassionate and dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our post-acute care team. The PMHNP will provide psychiatric assessment, diagnosis, treatment, consultation, education, and follow-up care to patients in skilled nursing and rehabilitation facilities.
This is a full-time, Monday–Friday position. The provider will commute to multiple post-acute care facilities daily, typically visiting 1–2 buildings per day depending on patient census.
Primary Responsibilities

Conduct psychiatric evaluations and record medical histories.

Adhere to proper documentation and charting protocols.

Collaborate with supervising physicians and interdisciplinary care teams to develop and update individualized care plans.

Order and interpret diagnostic tests and imaging as clinically indicated.

Diagnose psychiatric conditions and discuss results with patients.

Prescribe and manage medications and other treatment modalities.

Provide patient education on mental health, wellness, and preventive care.

Perform minor procedures within scope of practice.

Report deaths and communicable diseases as required by law.

Refer patients to specialists when clinically appropriate.

Other responsibilities as assigned.
Education and Experience

Active and unrestricted State of Florida APRN license in good standing.

PMHNP certification required.

National Board Certification and current DEA license required.

Experience in post-acute or long-term care settings preferred.

Minimum 1 year of psychiatric experience preferred (training available for the right candidate).

Additional Qualifications

Experience with electronic medical records (EMR) systems preferred.

Bilingual (English/Spanish) a plus, but not required.
