Psychiatric Mental Health Nurse Practitioner (PMHNP)
Locations: Englewood, Port Charlotte, and Sarasota, Florida

Job Summary

We are seeking a compassionate and dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our post-acute care team. The PMHNP will provide psychiatric assessment, diagnosis, treatment, consultation, education, and follow-up care to patients in skilled nursing and rehabilitation facilities.

This is a full-time, Monday–Friday position. The provider will commute to multiple post-acute care facilities daily, typically visiting 1–2 buildings per day depending on patient census.

Primary Responsibilities

  • Conduct psychiatric evaluations and record medical histories.

  • Adhere to proper documentation and charting protocols.

  • Collaborate with supervising physicians and interdisciplinary care teams to develop and update individualized care plans.

  • Order and interpret diagnostic tests and imaging as clinically indicated.

  • Diagnose psychiatric conditions and discuss results with patients.

  • Prescribe and manage medications and other treatment modalities.

  • Provide patient education on mental health, wellness, and preventive care.

  • Perform minor procedures within scope of practice.

  • Report deaths and communicable diseases as required by law.

  • Refer patients to specialists when clinically appropriate.

  • Other responsibilities as assigned.

Education and Experience

  • Active and unrestricted State of Florida APRN license in good standing.

  • PMHNP certification required.

  • National Board Certification and current DEA license required.

  • Experience in post-acute or long-term care settings preferred.

  • Minimum 1 year of psychiatric experience preferred (training available for the right candidate).

Knowledge, Skills, and Abilities

  • Strong interpersonal and communication skills (verbal, written, and electronic).

  • Ability to work both independently and collaboratively within a care team.

  • Demonstrated critical thinking and problem-solving abilities.

  • Commitment to compassionate, patient-centered care.

Additional Qualifications

  • Experience with electronic medical records (EMR) systems preferred.

  • Bilingual (English/Spanish) a plus, but not required.
