JOB SUMMARY:

PeopleOne Health is a direct-pay primary care organization where staff focus entirely on delivering exceptional care, free from the administrative burden of insurance-based models. Our multidisciplinary care team includes a Clinical Pharmacist whose role is to enhance patient outcomes, reduce overall healthcare costs, and improve the patient experience. The Clinical Pharmacist works primarily in an outpatient family medicine clinic setting and is responsible for performing medication reconciliation, counseling patients on medications and health-related topics, and conducting drug utilization reviews. This role also includes actively supporting the provider team through regular pharmacy rounds, offering clinical guidance, and contributing to the achievement of clinical quality measures related to prescribing practices and evidence-based medication management.

ESSENTIAL JOB FUNCTIONS:

  • Provide comprehensive medication therapy management for a wide variety of patients with chronic health conditions
  • Work closely with other members of the healthcare team including physicians, advanced practice providers, nurses, behavioral health specialists, and others
  • Engage in a collaborative practice model with providers to initiate, adjust, and discontinue medications, order and monitor labs, and refill prescriptions.
  • Respond to clinical questions and concerns on a timely basis
  • Serve as a key clinical support person for executing MTM services and precepting pharmacy students
  • Develop, implement and continually update patient educational materials
  • Participate in clinical and operational process improvement and cost savings opportunities while ensuring compliance with federal and state regulations
  • Maintain all patient information and records
  • Provide adequate notes and documentation within the EHR or MTM platform to ensure proper reimbursement, patient safety, and to provide seamless management of patients amongst all staff.
  • Remains current on departmental policies, procedures, formularies, and regulatory standards
  • Participate in departmental quality improvement activities, including clinical opportunities as well as operational efficiencies in order to continually improve and evaluate pharmaceutical care as reflected by operational and patient care indicators.

  • Comply with all applicable legal rules, regulations, and procedures
  • Ability to travel to designated area health centers to maintain compliance and be an in-person resource to patients and providers.

EDUCATION & CERTIFICATIONS :

  • Doctor of Pharmacy Degree (PharmD)
  • Completion of PGY1 Pharmacy Residency Program, or higher, or equivalent experience in ambulatory care preferred (see below)
  • Current active pharmacist license in good standing in the state of Florida required, candidates with additional licenses may be preferred
  • Current CPR Certification
  • Preferred, but not required, candidate is bilingual in English and Spanish or proficient in medical Spanish


EXPERIENCE:

  • 3-5 years of pharmacist experience or completion of PGY1 Pharmacy Residency Program
  • Ambulatory Care Pharmacy Experience preferred
