Field Human Service Case Manager Compensation
- The pay for this position is 34.13 per hour
- Benefits are available to full-time employees after 90 days of employment
Field Human Service Case Manager Highlights
- Location: Miami Dade County, FL (33172, 33182).
- Required Availability: Mon-Fri 8a-5pm
Field Human Service Case Manager Responsibilities
- Conducts comprehensive evaluation of Members using care management tools and information/data review Coordinates and implements assigned care plan activities and monitors care plan progress
- Conducts multidisciplinary review to achieve optimal outcomes
- Identifies and escalates quality of care issues through established channels
- Utilizes negotiation skills to secure appropriate options and services necessary to meet the member's benefits and/or healthcare needs.
Field Human Service Case Manager Requirements
- Travel up to 75% for member-facing visits and documentation.
- This position requires proficiency with computer skills, effective communication skills, and the ability to multitask in a fast-paced environment.
- Requires a High School Diploma or GED.
- Fingerprinting is required for this position.
- Position is in-person and patient-facing.
- Preferred Qualifications:
- Bilingual Spanish/EnglishAbility to multitask, prioritize and effectively adapt to a fast paced changing environmentEffective communication skills, both verbal and written
- Case Management Certification (CCM) preferred.
- Bachelor's degree required - No Nurses. Social Work degree or related field.
