Direct Support Professional (Direct Care Staff)

Under the supervision of the administrator, the direct care Staff will work closely with the clients and are responsible to provide a safe, secure, and healthy environment for all Individuals; uphold the principles of Person Centered Planning and always address the basic health, rights, privileges, and safety for all; interact closely with the clients; provide direct care as needed and required; adhere to all the policies and procedures of Charlies Angels Home Care Solutions, LLC at all times and overall daily routines; follow the Person Centered Plans for each resident and work with and assist each individual in their aim to achieve their individual goals.

Position: Direct Care Staff or Direct Support Staff

Qualifications:

1. Must be at least 18 years old

2. High school diploma or GED

3. Must complete all pre-service and in-service courses

4. Must be able to lift up to 105lbs

5. Must have valid Driver License

6. Bilingual is a PLUS

Pay Ranges: Starting at $15.00 / hr

Conditions of Employment

1. Clear of having been administratively determined to have committed abuse or neglect by a federal, state, or local government.

2. No criminal history or have a prior criminal history that may potentially be unacceptable for this field.

3. Present at work on time and when required.

4. Read and follow all the guidelines pertinent to the responsibilities of a Direct Care Worker.

5. Communicate with clients in a calm manner

6. Communicate well and in a team-spirited manner for fellow co-workers

7. Take direction well from Supervisor/Team Leaders, other Management, and Corporate Personnel.

General Description:

All Direct Care Staff provide direct, hands-on services to the client in various locations. Services may include any and/or all of the following:

Responsibilities:

1. Careful and complete adherence to the Person-Centered Plans/ Treatment Plans/Behavior Plans as written for each resident. Assist consumers with their efforts to achieve their personal goals and objectives.

2. Provide quality care, a safe and healthy setting that is free from harm, and with a mindful upholding of the rights of all clients.

3. Protect all clients from abuse and/or neglects and immediately report any occurrence to immediate supervisor and/or appropriate authorities as it happens or when it is discovered

4. Follow and uphold all house guidelines

5. Administer medications and maintain strict adherence to Medication training and agency guidelines.

6. Resident interaction required – activities, conversation, and training in Daily Living Skills as needed in all areas.

7. Complete all documentation prior to the end of each shift.

8. Assist resident in shopping and meal preparation. (if applicable)

9. Assist clients, as needed, in doing their personal laundry

10. Coach clients to maintain daily bedroom cleanliness per house guidelines or department training guidelines.

11. Maintain cleanliness off facility

12. Be physically able to assist in lifting of clients and be able to place items on higher shelves without restriction.

13. Advocate for the clients care, safety, and welfare as an active member of the tam of the person served.

14. Assist in the on-going development and implementation of each person’s training programs

15. Document all activities through progress notes and data collection as outlined within the individual treatment plan.

16. Attend and participate in regular staff training and scheduling meetings.

17. Conduct meetings with the clients

18. Maintain confidentiality and privacy regarding all clients’ information.

19. Respect the rights of the client and the client’s family.

20. Complete daily documentation of all agency required material.

21. Complete incident reports and accordance with policy and procedures.

22. Understand and follow the chain of communication.

23. Work cooperatively with other team members to carry out the goals and objectives of the client.

24. Serve as a liaison between the agency and the community in daily activities with clients.

25. Ensure the safety and well-being of the client on a daily basis.

26. Work cooperatively with other team members carry out the program goals and objectives for each client.

27. Develop and foster a professional relationship with the client based on mutual respect and trust.

28. Report any actual or suspected abuse/neglect in accordance with policy and procedure

29. Other client specific duties as assigned by the supervisor.

30. Maintain nutrition environment within the boundaries

31. As needed, provide crisis intervention to clients which may require unusual hours

32. Prepare meals ensuring nutritional content by following the facility menu.

33. Report any changes to consumer’s emotional or medical condition immediately to the facility administrator.

34. Wash and iron clothes as needed.

35. Maintain a clean home, dusting and mopping daily.

Work environment

Work is usually performed in a home-like setting and in community facilities with the potential of exposure to infectious disease. Universal precautions are required to minimize the risk of infections. Work sites may include locations that are not barrier-free. Duties are performed in an environment where the potential for physical aggression from clients may exist.
