Growing HOME HEALTH AGENCY in Hialeah, FL has an immediate FULL TIME position available for an experienced INTAKE CASE MANAGER to work in our office in Hialeah.

Must have HOME HEALTH EXPERIENCE and worked in a Home Health Agency.

We seek a dependable and self-motivated professional to join our staff. Must be fluent in English a must.

Clinical education and background a plus.

Job Requirements:

-Must be able to communicate fluently in ENGLISH with patients, family member's, Insurance companies, MD offices and field staff effectively

-Create and obtains all necessary physicians orders.

-Coordinate staffing of all disciplines and create schedules

-Must Be DETAIL oriented and organized with computer skills
