Job Title: Bilingual LTSS Care Coordinator (English/Spanish)
Location: North Miami, FL Must live in or near the follow zip codes (Zip Codes: 33012, 33013, 33014, 33015, 33167, 33147, 33142, 33150)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST
Reimbursement: Mileage reimbursement provided for travel from home to members’ homes and back
Pay rate $34.13 an hour
About the Role:
We are seeking a self-motivated, energetic, detail-oriented, and highly organized Case Management Coordinator to join our Case Management team. In this role, you will coordinate the care of our members, both telephonically and face-to-face, assessing, planning, implementing, and monitoring case management activities to promote overall wellness. This position allows autonomy and flexibility while providing meaningful impact on our members’ healthcare journeys.
Key Responsibilities:
- Coordinate case management activities for Medicaid Long-Term Care/Comprehensive Program enrollees
- Conduct comprehensive evaluations of members using care management tools and data review
- Implement and monitor assigned care plan activities, ensuring progress toward goals
- Facilitate multidisciplinary reviews to achieve optimal outcomes
- Identify and escalate quality of care issues through established channels
- Use negotiation and motivational interviewing skills to secure services and promote member engagement
- Provide coaching and support to empower members to make informed healthcare decisions
- Collaborate with PCPs, skilled providers, and community resources to address member needs
- Ensure compliance with case management, quality management, regulatory, and accreditation guidelines
Qualifications:
- Must be bilingual in English and Spanish(fluent in speaking and writing)
- Minimum one year of case management experience (long-term care experience preferred)
- High School Diploma or GED required; Bachelor’s degree preferred (Social Work or related field)
Preferred:
- Case Management Certification
- Experience with Medicaid Long-Term Care programs
