Position Title: Physical Therapist PRN (Weekend Coverage)

Job Summary
We are seeking a PRN Physical Therapist to provide weekend coverage while delivering compassionate, patient-centered care. Our team takes pride in building meaningful relationships with patients and delivering high-quality therapy that promotes independence, safety, and quality of life.
The PRN Physical Therapist evaluates and treats patients in accordance with physician orders, using therapeutic interventions to address deficits in physical function. This role requires efficient, autonomous clinical decision-making while collaborating with the interdisciplinary care team and supervising Physical Therapist Assistants (PTAs) as needed.
Essential Duties & Responsibilities
- Provide physical therapy services during weekend coverage as scheduled
- Respond to and initiate patient referrals
- Review patient medical records and evaluate their impact on assessment and treatment
- Perform evaluations and develop individualized treatment plans with functional, measurable goals
- Implement treatment plans and modify interventions based on patient progress
- Complete accurate, timely documentation, including evaluations, daily notes, and discharge summaries
- Supervise and direct Physical Therapist Assistants when applicable
- Educate patients and caregivers on treatment plans, exercises, and post-discharge recommendations
- Communicate effectively with nursing staff and the interdisciplinary care team
- Participate in quality improvement and compliance initiatives as needed
- Perform other duties as assigned

Essential Physical Requirements
- Ability to move intermittently throughout the workday
- Ability to lift over 50 pounds with assistance, based on patient needs
- Ability to sit and walk for extended periods
- Ability to bend, squat, push, stoop, and pull to provide patient care
- Must meet general health requirements per facility policy, including completion of a physical examination
