We are seeking a Supervisor, Care Managers to join our team at Independent Living Systems (ILS). ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations.

About the Role:

The Supervisor, Care Managers plays a pivotal role in overseeing and guiding a team of care managers to ensure the delivery of high-quality, member-centered care within the healthcare and social assistance sector. This position is responsible for coordinating care management activities, monitoring case progress, and ensuring compliance with organizational policies and regulatory requirements. The supervisor will foster a collaborative environment that supports professional development and effective communication among care managers, members, and interdisciplinary teams. By analyzing care outcomes and resource utilization, the supervisor will implement strategies to improve service efficiency and member satisfaction. Ultimately, this role ensures that care management services align with best practices and organizational goals to enhance overall health outcomes for the populations served.

Minimum Qualifications:

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
  • Minimum of 3 years of experience in care management or case management within a healthcare or social assistance setting.
  • At least 1 year of supervisory or team leadership experience in a healthcare environment.
  • Strong knowledge of care coordination principles, healthcare regulations, and member privacy laws (e.g., HIPAA).
  • Proficiency in electronic health records (EHR) systems and care management software.
  • Relevant experience may substitute for the educational requirement on a year-for-year basis.

Preferred Qualifications:

  • Master’s degree in Nursing, Social Work, Healthcare Administration, or a related discipline.
  • Certification in Case Management (CCM) or related professional credential.
  • Experience working with diverse member populations and complex care needs.
  • Familiarity with quality improvement methodologies such as Lean or Six Sigma.

Responsibilities:

  • Lead, mentor, and support a team of care managers to deliver comprehensive care coordination and case management services.
  • Monitor and evaluate care plans to ensure they meet member needs and comply with clinical guidelines and regulatory standards.
  • Facilitate communication and collaboration among care managers, healthcare providers, members, and families to optimize care delivery.
  • Conduct regular performance reviews, provide feedback, and identify training needs to promote continuous professional growth.
  • Analyze care management data and outcomes to identify trends, address challenges, and implement quality improvement initiatives.
