Job Summary
We are seeking a Licensed Mental Health Counselor (LMHC) or Psychologist to provide compassionate psychiatric and behavioral health services across multiple post-acute care facilities. This role involves commuting between sites, conducting patient rounds, collaborating with facility staff, and delivering assessment, diagnosis, treatment, education, and follow-up care. The provider will work alongside members of the Psych Division within post-acute settings.
A current Florida license is required.

Primary Functions

Conduct patient examinations and document medical/behavioral health history

Adhere to proper charting and documentation protocols

Collaborate with physicians and healthcare professionals to develop care plans

Submit care plans and patient goals for periodic physician review

Educate patients on wellness, hygiene, diet, and prevention strategies

Refer patients to specialists when appropriate

Report deaths and communicable diseases according to state guidelines

Perform additional duties as assigned
Education & Experience

Active and unrestricted State of Florida license in good standing

Must be an LMHC or Psychologist in Florida

Current National Board Certification
Knowledge, Skills & Abilities

Strong written, verbal, and electronic communication skills

Demonstrated initiative, creativity, and problem-solving ability

Ability to collaborate effectively with interdisciplinary teams
Additional Qualifications

Experience with electronic medical records (EMR) preferred

Bilingual (English/Spanish) a plus but not required
