Position Summary
We are seeking a Physical Therapist to provide high-quality, evidence-based physical therapy services to patients with acute injuries and chronic musculoskeletal conditions. This role delivers care primarily onsite, with occasional virtual services as needed, and focuses on improving patient mobility, function, and long-term wellness through individualized treatment plans.
Key Responsibilities

Provide onsite physical therapy services using evidence-based therapeutic modalities

Deliver virtual physical therapy services when clinically appropriate

Evaluate and treat patients with acute injuries and chronic musculoskeletal conditions

Develop, implement, and monitor individualized home exercise programs

Support patient adherence to prescribed treatment and exercise plans

Assist patients in making sustainable lifestyle changes to improve overall health and functional mobility

Educate patients on injury prevention, ergonomics, safety, and chronic disease management through movement strategies

Serve as a subject matter expert for early identification and self-management of musculoskeletal conditions

Reduce unnecessary specialty and imaging referrals through early intervention and patient education

Collaborate with interdisciplinary providers and community resources to ensure continuity of care

Act as a liaison between patients, healthcare providers, and support services within the care network

Maintain accurate clinical documentation in accordance with professional standards
Qualifications

Doctor of Physical Therapy (DPT) preferred

Active Physical Therapist license in the state of practice (or eligibility to obtain)

Experience treating musculoskeletal and chronic medical conditions preferred

Experience working with diverse patient populations is a plus

Strong communication, clinical reasoning, and patient-education skills
