ABOUT THE ORGANIZATION

Established in 1970, Boley Centers, Inc. is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County. Boley Centers has an opportunity for a full-time Supported Housing Team Coordinator to provide direct services to consumers such as assessment, service planning and recovery services. The Team Coordinator will also act as manager in his or her absence. to ensure prompt responses by the team to crisis situations, tracks services and documentation and assist the Manager in training and reviewing the work of team members.


DUTIES/RESPONSIBILITIES:

  • Works with staff to assure quality services that are coordinated among team members as needed. Reviews documentation of services delivered. Trains team members. Participates in quality improvement activities such as reviewing the quality of service delivery, documentation of services and participates in Agency meetings that address quality service delivery.
  • Acts in the absence of the Manager to coordinate the services provided by team members. Assigns staff, as needed. Leads team meetings. Coordinates crisis intervention.
  • Provides direct services including bio-psychosocial assessment, recovery planning, case management, crisis intervention and housing supports.
  • Builds relationships with all consumers assigned to the team in order to provide effective services to all individuals.
  • Assists consumers in assessing an array of resources in the community such as entitlements, medical care, social activities, employment and other services.
  • Teaches consumers skills such as transportation, budgeting, medication management, social and leisure skills.
  • Supervises Recovery Coaches.
  • Transports consumers, as needed.
  • Monitors supported housing facilities assigned to the team.

EDUCATION AND EXPERIENCE:

Bachelor's degree required; human services or related field preferred. No substitution permitted. Education must be from an accredited school, college, or university.

Three to four years relevant experience in human services with some experience working with mentally ill adults preferred. At least one year of supervisory experience required.

SPECIAL REQUIREMENTS

Valid Florida driver's license, insurance and own transportation required.

Must pass a Level II background check.
